Exposure and Response Prevention is the treatment with the strongest evidence behind it for OCD, and it works on the loop rather than the thought.
Exposure means going toward the thing you are afraid of, on purpose and repeatedly, instead of away from it. Response prevention means not doing the thing that makes the discomfort stop — not checking, not washing, not asking, not going back over it in your head.
That is the whole mechanism. You feel the discomfort and you do not fix it. Nothing is neutralized, nothing is argued with, nothing is proven safe. And over enough repetitions the alarm stops being worth sounding.
It is uncomfortable and it is meant to be. That is not a side effect of the treatment. It is the treatment.
